The Hospice of Volusia/Flagler has been providing End-of-Life Care in Volusia and Flagler counties since 1979 and works hand in hand with nursing home facilities to serve mutual residents.

Port Orange Nursing and Rehab Center stood out as having achieved excellence in areas of this mutual undertaking to provide Quality of Life, Dignity and Comfort for their terminally ill residents.

Having met the criteria, they were awarded the Partnership in End of Life Care plaque on September 19th, 2007.
