The Program Development Team is available to assist hospital systems and physicians in creating patient service programs to meet their discharge needs. Once the clinical need has been established and one or more of our facilities have been identified to provide the service, our team will train our personnel and obtain the needed equipment and supplies in order to provide the right level of care based on individual needs.

We are currently working with several hospitals and physicians to provide the following services:

	In-House Dialysis Care
	Heavy Wound Care
	Bariatric Related Care
	Orthopaedic Recovery Progam
	PACER (Post Acute Cardiac Event Recovery Program)
