Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

Food was good not what you’d expect from a rehab facility. [review] => This place is the absolute finest of the finest! Donna V. and her team there are so wonderful. We had to place my mother in a long term facility and after visiting many different bad facilities of no help, smelly halls, mean staff, and unhappy residents, we came to visit Port Orange Nursing and wow! It was so overwhelming with lots of love not only from the staff but the residents there are happy too! When you walk in, you can feel the a/c, theres no smell of urine, the staff is smiling, and….. the residents are extremely happy. What a slice of heaven! I would highly recommend this facility to anyone considering or needing to place their loved one in special care. Thank you Donna for such a great experience!!!

[review] => Please DO NOT send anyone you love to Port Orange Nursing & Rehab. I made this horrible mistake and live with regret. Everyday.
Yes, the facility looks tidy enough, the small outdoor garden area is lovely and if you make a visit prior to admittance your tour guide will be pleasant and promise good care. Once your loved one is there, they will experience what the tour did not reveal. Call buttons that go unanswered by the nurses station. Literal cries for help that are ignored. Imagine visiting your loved one in the morning and learning that they spent hours in a urine-soaked, or worse, adult diaper. Report a grievance, but do not be surprised when nothing happens. And when the poor person in the next bed is “isolating” from a mystery (staff cannot tell you) illness, don’t expect that curtain in the semi-private room to provide protection to your loved one. It won’t. Port Orange Nursing and Rehab did not kill my father, who was suffering from multiple, serious ailments. But in his brief stay, he was rarely treated with the professionalism and empathy that could have potentially helped in his recovery or at least provided peace and dignity in his last days.

The staff are all cordial and friendly. The therapist's do a fantastic job. My Dad was there for about 3 weeks, and even with his many complicated issues, they were on top of everything. [review] => My mom was in this facility and all I can say is it was an unpleasant experience for her. The service was terrible along with the food. When she rang for someone to come and assist her it sometimes took them 45 minutes to an hour for someone to come to her room. Some of the staff was rude to her expected her to get herself up and out of the bed with a broken hip. That was totally unexceptable. Finally, she was telling us her wound was leaking and all the staff did was remove her pajama bottoms. When we looked at the staples we could see that it was infected. Needless to say the nurse never checked her wound and did not know it was infected. We sent a picture to her dr. And she was then placed on an antibiotic. When she was discharged and being transported home there was to be a walker available for her either when she left the facility or at her home. That did not happen. They let her take a walker from the facility and wanted it returned back to them once she arrived home. How did they expect her to get around without that walker. Finally all of her PT and OT was to be arranged before she left the facility and that did not happen either. I had to call 3 days after we arrived home to find out when someone would be contacting us to set up the visits. I was then told it had just been approved that day. We were very unhappy with this facility and would not recommend it to anyone.

While it may look nice, it's poorly staffed, they don't follow medical procedures properly and they do not pay their staff I a timely manner and the staff turnover rate is excessive high. Go in any time of the day and see how many residents are in soiled undergarments because the cans can't be found. I got my mom the hell out of here. Please research this place before you sign anything!!!!!

[review] => My husband spent almost 10 days in this facility and although the nursing and PT staff were very pleasant, the care was less than par. They brought one cup of water a day and the meals were horrible. He was supposed to be on a schedule for his medication after having two very painful surgeries and had to wheel himself down to ask for it because it was hours overdue. The gentlemen who shared his room was an insulin dependent diabetic and never received his insulin on a consistent basis. The front desk staff are rude and condescending. This was a terrible experience after an already difficult ordeal. Don't leave your loved ones unattended in this facility.

My grandmother was placed at this facility after a femur break… she was bed bound for 4 weeks. The nurses were absolutely horrible to her. She would call us crying every night because she was so upset by her treatment. The male CNA would take her remote and call button and put it above her bed where she could not reach it so she couldn’t call them for help. She asked for help with tasks and was told by nursing staff “we aren’t helping you do anything you can do yourself”…
her rooommate who is the same age would also tell us about the mistreatment she experienced. She would push the call button for help using the rest room and they wouldn’t come in time and be angry at her for having an accident.
The front desk staff is rude and lazy, they said they wouldn’t bill her insurance for transport to her doctor for a checkup so we had to pay $140 out of pocket.
She now has a bedsore that is the entire size of her bottom…as an emt it is the worst bed sore I have ever seen. It’s taken an arm and a leg to get them to care for it. She is in so much pain.
The ONLY redeeming thing about this place is the activities department, they are super sweet and my grandma had fun when she was playing bingo.

[review] => My wife spent 34 days there. The PT & OT were excellent.
Two days after she got there, someone changed her prescription medication for IBS from 3 times a day to as needed and gave her stool softener.
This caused her daily digestive distress.
We only learned of this the day before she was discharged.
There were 3 large cockroaches in the bathroom at separate times.
The nurses were very good to excellent.
She asked for a shower numerous times but only got one. A roommate got 2 or more every week.
The food was not up to par.
We were disappointed by the total package.

[review] => I have updated my review simply for the staff. Everyone they have on staff is very good. Rehab staff is excellent. There should be a curfew for TV’s. Some patients are hard of hearing and have their tv’s up loud. Even with closing my door it was still loud. Some of the patients are asleep and tv still going. Different channels on loud. Very hard to sleep. Last night there hubby brought me ear plugs. Thank you to all the CNA’s.

[review] => My experience at the rehab center was absolutely fantastic the nursing staff has been wonderful I've been here for two and a half months and they've treated me with nothing but respect places clean and well efficient I recommend it to anyone who needs help with rehab and or nursing care. My deepest appreciation for this facility and two nurses in particular Brittany and Oralene. Thank you ladies for everything I hope that facility treats you well as you treated me.

[review] => I was referred to Port Orange Nursing and rehab center by the hospital. I'm fortunate enough to have used their facility twice and I'm a better and ;healther man because of their help and expertise. The entire staff does their best to make you feel .comfortable and well taken care of. The rehab program is excellent and I spent a lot of time. there The facility is clean and the rooms are nice and the food is good. The chef sends a weekly sheet for you to alter your menu for your own preference. A very easy place to refer to any friends or relatives that need help after they're out of the hospital. Alex

[review] => Facility is clean and a variety of staff visit each day to check on you. Therapy is superior. Meals are adequate. Only real complaint is response time to call bells which needs to improve. Also that some staff have commented "it's not their job" when requesting help for different things. More teamwork among staff is needed. Also more communication advising patients about communion being available for Catholics, how to fill out meal requests in advance, etc. But I would come back.

[review] => I have been a resident at Port Orange Nursing and Rehab for one year. For the most part it has been a very positive experience. I love the activities they have here. They keep me busy in an entertaining way, especially the live music. The nursing staff is good to excellent and the CNAs work hard to keep us comfortable. There is a beautiful tropical garden with a water fountain that I spend time in nearly every day and is quite relaxing. The food they serve to us is OK and it is sure nice to have it brought to us and picked up each day. The administrators are quite good at what they do. The Physical and Occupational Therapists are excellent and although it is hard work they make it fun for us. If I need help in the future I will be returning to Port Orange Nursing and Rehab Center. [review] => My Dad fell and after being in the hospital for 8 days needed a rehab facility I chose PO rehab since he had been there before and it was close to home. However he tested positive for Covid and was sent to another facility which is another story. This facility is clean and looks homey. The staff is friendly although on weekends staff is limited. He said food was better than other place so the weight he lost in the hospital he put back on here plus he could pick his menu which was a help in getting him to eat. When the patient gets to this facility staff meet with the family and patient to get to know each patient and their needs. Of course I had a couple of things I asked about during his stay but overall my Dad had a good stay and is home continuing to recover!!

When I spoke to the DON I jjust got excuses instead of I’m sorry this happened. The woman. In the office sat there eating Wendy’s chili as we spoke to her to close my mothers account. The lack of empathy and caring here is amazing I would not even recommend this place to my worse enemy.
Added 1/24: Actually I did call to speak to someone and I believe it was the DON. Stacey. I spoke to the admin as well about a week later. There was only excuses about why it wasn’t your policy to call a patient’s family once they admitted to hospice. My mom has not even been fully evaluated to be admitted yet, although yes, I had signed the papers the previous day. Someone could have called myself or my father that night when she got worse. Somebody from your facility could have called me when she died, but you didn’t. I found out a half hour after from the hospice nurse who had never even seen my mom.

This is due to employee's failing to follow protocol. Then they terminated the Infection Prevention Control Officer after she returned from BRAIN SURGERY because she was limited to 4 hours per day and was having side effects. Tim Leonard, you should be removed from your position as Administrator immediately.

[review] => I recommend against allowing a loved one to stay at this rehab facility. We sent my elderly father here in early June 2022 on recommendation of the hospital that performed his hip replacement surgery. Most of the staff we encountered were independent contractors, and their rehab knowledge and skills were marginal. When I visited my father, he was dehydrated and incapable of eating. However, no one could find the head nurse or attending physician to set up an IV. I could go on and on. Ultimately, my father died in this place.

I witnessed a total lack of personal care by most of the staff, which most seem to be contractors that are only doing their time. From nutrition to therapy, even as the patient slept, a person came in to ask if it was time to exercise.....he was asleep!! They never took note of how much food was ingested, as another meal was served, with the uneaten meal removed. All the while knowing the patient was not eating or drinking any fluids. We even had a head floor nurse claim she is not going to tend to a covid patient, that is not what she was hired for?? Bring your loved one to a place that truely cares, monitors their patients well being 24/7.

Staff spent tons of time congregating in the hall and pretty much ignored my mother in law’s and her roommates needs. We are removing her from the facility in less than two days there and we will have to get her home care at our cost. We also have an advocate from a private agency which we are paying for to stay with her to assure things get done until we move her home. They should be paying for our advocate! We decided it was best after 12 days in rehab to go home and have Home nurse and Therapy.

[review] => I am happy to give Port Orange Nursing and Rehab Center 4 stars. After a long, drawn out bookkeeping issue, I did not wish to give them 5 stars. My 95 year old mother has been a long term resident at this facility for a year now. We are grateful for the care she's been given, since being a long term resident on the East Wing. Her Nurses and CNAs always treat her with kindness, care, patience, and top notch skill. They notify me promptly when Mom has a fall, skin tear, or other problem, no matter how small. The Activity Department, especially Leslie, is always attentive to Mom and has even sent me photos of Mom if we're out of town. We visit Mom 3 days a week and the facility is always clean and pleasant to visit. Other residents are friendly and appear to be as content as Mom. Mom's clothes are laundered promptly, and she is always clean and well dressed. Colleen, in the Salon, has done a fabulous job with Mom's hair each week. Chantel has been a wonderful asset to us, right from the start. Stacie and the entire PT,OT, and Speech Dept. has helped Mom retain as much of her physical abilities as possible and continue to monitor her. We are grateful to have Mom is this facility and know that we can go about our own daily needs, knowing that Mom is well taken care of.

The activities Director is an example of going beyond the call! The facility, inside and out, was always meticulously clean. [review] => This is a clean and well maintained building. However the patient care is not good. My mother-in-law was given poor care during her stay for rehab. , following a hip repair. After being there for 4 weeks she could barely stand, and pivot. She developed a pressure sore on her foot on the operated leg, limiting her rehab. It took a week for that to be addressed. She was left in the wheel chair all day causing her leg to swell. As her condition deteriorated she was not assisted to eat. I can not recommend this facility for elderly care. It may be fine for someone who is alert and capable.

[review] => I have been a patient here for a week now, with severe neuropathy in my legs. Since my admission the entire staff treated me like I was part of the family. I have been doing physical therapy daily. The nursing staff dispenses in my medication in a timely manner. The food is good and you can order a substitute meal if needed.
It's the holiday season and the staff is cheerful but not overbearing. I would encourage anyone needing medical attention to contact the facility.
Lastly, supervisor Wendy helped me with a special request. It's a great place to get it together and regroup

Food was not appetizing or appealing. Rehab consisted of one 30 minute session a day. He was laying in bed the rest of the time for the most part. How do you learn to walk again like that. We pulled him out and moved him to our hometown 12 hours away it was that bad. He went from hell to heaven in the rehab center we got him here. There has got to be a better place in the Daytona area than this place. With all that said there were a couple of very kind nurses. I think they were mentioned in another post.
** just following up that the Director reached out to me which I appreciated. It seems they are not ignoring these issues and are making efforts to correct them and improve. I raised my ranking to a two star because of the directors effort to learn what the issues are. I would go to a 2.5 but just can't get to the 3 star.

[review] => If there was a rating with a negative star that is what i would selected. To some up care ZERO. For.one example of care my mother has received. My father had to point out to staff of an open wound on my mother's back.
Then a couple times my father witnessed staff placing a Soiled wound bandage back on the open wound. My mother is diabetic, which complicates her healing and makes her more prone to infections. The list could go on for the treatment both my parents have recieved from this facility. When my father spoke up about the treatment my mother is recieving he now has limited visitation. Don't send your loved ones here.

[review] => I asked the director of physical therapy how my family member would be able to recover and get stronger if he was in bed 23 hrs a day on opiods and sedatives. I told her I was hoping he would be exercising and getting stronger from more PT (aft a fall). She replied “we don’t do that here”. My parent came out with muscles weak and withered.
Also, lots of very grumpy ppl here who seem to hate their jobs. Beware they may not have time to return your call(s).
The best thing that happened at this place is we learned to avoid it. A few people are excellent here including Chantel, Michelle, Karen. It’s too bad one of them doesn’t run the place.
Lastly, when you see positive reviews here – notice that they are often from people connected to this place by employment and not written by patients or patients’ family members. These people are not “customers” so why are they leaving reviews unless they were asked to? Smh.

[review] => This facility has to be one of the worse nursing homes in the area. Do not trust them with your loved ones. My mom was there approximately one month before she passed on. She would tell us everyday how she was treated so poorly. She would call for assistance and took so long for someone to come. She was bedridden and we relied on them to take care of her needs. One day she said they left her in a wheelchair for three hours. I did call and speak to them about another incident that an aide scolded my 92 year old mother because she needed help. That was while her granddaughter was on the phone to hear that conversation. In her final hours we were told that myself and my two daughters could not be by her side due to COVID restrictions. The day prior wasn’t a problem though. Lack of communication with the family and the rude treatment for the patient and family is deplorable.

[review] => DO NOT trust your loved ones at this poorly managed facility! My Grandmother was placed at this facility for approximately one month and it was nothing but a nightmare from day one. We had no communication from the staff about her care and overall health, which is pretty essential when you cannot visit the facility due to COVID lockdowns. We trusted this facility and staff after careful consideration and much research on nursing homes in the area, only to majorly regret this decision four weeks later when she passed away. The nursing staff was rude, paid little to no attention to the residents, and even left my grandmother in a wheelchair for four hours and unable to get back to her bed after "forgetting" to take her for a walk after getting her up. They then tried to deny my family to be at her bedside in her final hours due to COVID restrictions, even though we had already been there the previous 24 hours. There was no consistency in guidelines from day to day and the care she received was subpar at best. She was very unhappy and upset everyday when we spoke to her and we could never get anyone from the facility on the phone to talk about what was going on. She would tell us she hits the call button in her bed when she needed help and nobody would ever come to assist. She was bed ridden at this point and heavily relied on the staff for assistance. While on the phone with her one evening, I even witnessed an aide scold her for calling for help and bothering them, which is what they are getting paid to do and the whole reason she was there!! I would not trust my dog to the care of this facility, let alone another family member.

[review] => My dad was put in this gosh awful place no sanitary service bed was never changed until I asked them to do it food was very small in portions he is 6’6 needed bigger portions we asked said they would accommodate but didn’t bathroom and floors were so dirty staff was ok but when you ask for something it’s like you just asked them for the world and they give attitude NEVER AGAIN this was just for rehab!!!! Thank god I am taking him home today and WOULD NEVER RECOMMEND THIS PLACE FOR ELDERS! They deserve to be treated better than this!!!!!

[review] => This was only a temporary situation for my mother. It seems to be like a few other nursing homes in the area. Communication is not great, but better than at the last rehab center my mother visited. This place is leaps and bounds above that place (which I won't mention by name at my mother's doctor's request). I like the outdoor fountain and gardens which is a nice area to take loved ones during visits.

I will not recommend this place for anyone that have a family in need of rehab services, please due your research before admittting your love one to this place.
The staff gets upset for what they are getting paid to do as a career they chose. They do not have the heart or passion to do this job!!! I forgot to mentioned the therapist Irma was cruel pushing my mother to do more than what she can handle. Also the staff are negligent about bathroom needs.My love one has suffered to the point that does not want to ask for help, due to their rudeness and lack of professionalism.
Please DO NOT even consider or give this place a chance.

[review] => This place is very impressive to family and visitors, because it's daytime. I was in Rehab in November 2014. Therapy was great, though way too overcrowded. Meals were ok. Once the doors locked at night, the place turned into a nightmare. I had a LPN threaten me at 2am, while I had no roommate to witness. I was told that I might not want to rock the boat while was here. Then refused to let me reply, kept telling me she wasn't finished. Spoke with Administrator, who did nothing. Went to another facility in Daytona, SO much better.

[review] => They drugged my mother up so much she couldn't feed herself and couldn't speak. Transferred her to another facility and had her back talking and feeding herself in 24 hours. Almost lost the Medicare days for rehab because of this place. Most CNAs were nice but overworked not enough of them for the patients. Dirty laundry left in the room and gloves on the floor. I would give 0 stars to this place! The admissions lady could have cared less about anything we talked to her about. My advise would be to do more research on a place before you place a loved one in any care center.

My grandmother is so uncomfortable and angry with the staff here and she's by all means NOT the angry kind of gal! I'll change my review when the staff changes there attitude! Your job has to do with other human beings quality of life! If you can't respect that, please try for a new profession!

My dad spent 20 days at this facility the staff were all very attentive listened to what my dads needs were & followed through the entire stay. Food was good not what you’d expect from a rehab facility. Special thanks to Kenya one of the charge nurses she is amazing & takes especially care not only for the residents but also for her other staff. She is professional in every way. Thank you!

This place is the absolute finest of the finest! Donna V. and her team there are so wonderful. We had to place my mother in a long term facility and after visiting many different bad facilities of no help, smelly halls, mean staff, and unhappy residents, we came to visit Port Orange Nursing and wow! It was so overwhelming with lots of love not only from the staff but the residents there are happy too! When you walk in, you can feel the a/c, theres no smell of urine, the staff is smiling, and….. the residents are extremely happy. What a slice of heaven! I would highly recommend this facility to anyone considering or needing to place their loved one in special care. Thank you Donna for such a great experience!!!
