Short-Term Stay After Hospital Admission

Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

                            [review] => I can honestly say this facility is one of the best facilities in the area. They have a proactive approach to your loved one's care. Upon entering the building, you will be greeted by Christina, the receptionist where she will give each visitor in your party a badge. I know sometimes there may be a line and you might have to wait, but that is only to protect you and your love one, in the event of an emergency, the facility has an accurate count of staff, visitors, and residents. The doors stay locked. You can press a buzzer to enter, collect a badge, and visit your loved one. Keep that badge because you will need it to get out. Visiting after hours, no problem. They have an additional buzzer, that goes directly to the floor. You might have to wait a minute because the staff member has to walk down to the front to let you in. When your loved one arrives for admission, they will be greeted by a nurse and a CNA. Be patient. It may take some time. This facility is busy. On average they are getting two to three admissions a day. A nurse can have anywhere from 1 to 40 patients while a CNA can have anywhere from 1 to 20. I can assure you this facility staff's well above the STATE guidelines. The CNA may have anywhere from 1 to 15 on a bad day and the nurse may have anywhere from 1 to 30. You may hear a staff member from time to time. Say we are working short today, That's not true. They are just spoiled because usually they have anywhere from 1 to 10 patients.
Your loved one will get 3+ meals a day. There is a selective menu that they can fill out a week in advance. If their meal tray arrives in the room and they don't like it, they can always ask for an alternative which usually includes grilled cheese, chicken tenders, a hamburger, or many other options. Tony, the dietary manager will come around and get your loved ones likes and dislikes. And while we still have to follow your loved one's diet, Tony will certainly try to include their likes. I've seen Tony go as far as making special meals for patients who are refusing to eat or losing weight.
Your loved one does not have to just lay there in the bed like a hospital. They do not have to just stay in their room. There is a wonderful activities department that is ran by Caitlin , Shannon, and Shirley. Everyday these ladies have some type of fun entertainment like live music, bingo, popcorn in a movie, coloring, arts and crafts and much more.

This facility has one of the best therapy departments I've seen. They have physical therapy, occupational therapy and speech therapy. All three departments. Have a wonderful therapy manager named Stacey. Stacy has been with the company for over 20 years. If you like the therapy, your loved one is receiving in the facility, once they are discharged you can bring them back for outpatient therapy.

Looking to arrange a discharge? You can talk with Bob or Candice from social services. They usually set everything up for you. Whatever equipment you need upon discharge,  to your next doctor's appointment. Want your loved one to stay long-term care or transfer to another facility. They can help with that too. Missing an item or have a complaint. Have a staff member help you fill out a grievance. They usually find and return the item quickly. Or as long as it's on your loved one's inventory sheet, the facility will cover it.
Trayshawn in the business office is good at notifying you of your loved ones co-pays or insurance updates. She can also help with the Medicaid application process.
The management team here do a phenomenal job. They ensure that your loved one has the best care possible and that all policy and procedures are being followed. However, they are not mind readers. They do not know something is going on unless you speak up and tell them. I can assure you they are quick to resolve whatever issues you or your loved one is having.
My time has come to an end at this facility, by my own choice, but I can assure you, your loved one is in good hands!

                            [review] => During the time my wife was a patient there for 180 days overall the care was very good to excellent depending on the treatment provider.  Considering the length of stay there will be  both good and not so good experiences but neither should solely determine the final result of returning home and feeling better.  It seemed that the CNA’s who carried a lot of the workload had to many patients and not a lot of time to check on them but once or twice during a shift. However, they did fine.  Tho PT/OT staff was outstanding and deserve much credit for their commitment and professionalism in restoring her functionality.  Over this lengthy stay we met and interacted with so many wonderful and caring staff that to try and name all and miss one would be a disservice to the omitted and an embarrassment for us.                             [review] => we chose here because the hospital recommended. I’m gonna say the facility is very clean however and I know this is most all facilities. The beds have minimal comfort and the nursing care is not the greatest. They would put my father in a wheelchair every day for over eight hours, and if I didn’t go there every day at 3 o’clock to ask them to put him in his bed. He would remain there until well after dinner. I’m much younger and I couldn’t sit in wheelchair that long,  place has no recliners. It’s really meant for short short term rehab, he pretty much sat in his room, all by hisself , staring at the walls or they would stick them across from the Desk for the day, the rehab was very minimal. they were really good about changing the bed and bringing fresh water twice a day. The food was pretty good but as far as safety, He fell twice. I do not feel that they kept him safe. There are no railings not even at the top of the bed to grab and help roll over or keep patients from falling out of bed I found this quite strange. He also fell out of his wheelchair so obvious he was not safe. I would only recommend this to somebody who needs minimal care and minimal rehab. If I could do it over again I would’ve brought him home and had physical therapy. Come to the house . After 20 days there,  They ended up calling an ambulance and sending out to the ER and from there he got worse and passed away

Yes, the facility looks tidy enough, the small outdoor garden area is lovely and if you make a visit prior to admittance your tour guide will be pleasant and promise good care. Once your loved one is there, they will experience what the tour did not reveal. Call buttons that go unanswered by the nurses station. Literal cries for help that are ignored. Imagine visiting your loved one in the morning and learning that they spent hours in a urine-soaked, or worse, adult diaper. Report a grievance, but do not be surprised when nothing happens. And when the poor person in the next bed is “isolating” from a mystery (staff cannot tell you) illness, don’t expect that curtain in the semi-private room to provide protection to your loved one. It won’t. Port Orange Nursing and Rehab did not kill my father, who was suffering from multiple, serious ailments. But in his brief stay, he was rarely treated with the professionalism and empathy that could have potentially helped in his recovery or at least provided peace and dignity in his last days.

                            [review] => My mom was in this facility and all I can say is it was an unpleasant experience for her. The service was terrible along with the food. When she rang for someone to come and assist her it sometimes took them 45 minutes to an hour for someone to come to her room. Some of the staff was rude to her expected her to get herself up and out of the bed with a broken hip. That was totally unexceptable. Finally, she was telling us her wound was leaking and all the staff did was remove her pajama bottoms. When we looked at the staples we could see that it was infected. Needless to say the nurse never checked her wound and did not know it was infected. We sent a picture to her dr. And she was then placed on an antibiotic. When she was discharged and being transported home there was to be a walker available for her either when she left the facility or at her home. That did not happen. They let her take a walker from the facility and wanted it returned back to them once she arrived home. How did they expect her to get around without that walker. Finally all of her PT and OT was to be arranged before she  left the facility and that did not happen either. I had to call 3 days after we arrived home to find out when someone would be contacting us to set up the visits.  I was then told it had just been approved that day. We were very unhappy with this facility and would not recommend it to anyone.

They brought one cup of water a day and the meals were horrible. He was supposed to be on a schedule for his medication after having two very painful surgeries and had to wheel himself down to ask for it because it was hours overdue. The gentlemen who shared his room was an insulin dependent diabetic and never received his insulin on a consistent basis. The front desk staff are rude and condescending. This was a terrible experience after an already difficult  ordeal. Don't leave your loved ones unattended  in this facility.

My grandmother was placed at this facility after a femur break… she was bed bound for 4 weeks. The nurses were absolutely horrible to her. She would call us crying every night because she was so upset by her treatment. The male CNA would take her remote and call button and put it above her bed where she could not reach it so she couldn’t call them for help. She asked for help with tasks and was told by nursing staff “we aren’t helping you do anything you can do yourself”…
her rooommate who is the same age would also tell us about the mistreatment she experienced. She would push the call button for help using the rest room and they wouldn’t come in time and be angry at her for having an accident.
The front desk staff is rude and lazy, they said they wouldn’t bill her insurance for transport to her doctor for a checkup so we had to pay $140 out of pocket.
She now has a bedsore that is the entire size of her bottom…as an emt it is the worst bed sore I have ever seen. It’s taken an arm and a leg to get them to care for it. She is in so much pain.
The ONLY redeeming thing about this place is the activities department, they are super sweet and my grandma had fun when she was playing bingo.

When you attempt to make a complaint she cuts the resident short and states that she's not sticking up for the staff......she does not offer a grievance notice for the resident to file. CNA's on 3-11 and 11-7 are nasty and rude to the residents will not did not supply water....did not change briefs....did not assist residents to the toilet but they do tell you to mind your business ....should a resident expire during there shift because of their neglectence then it's on them .  This facility has a rating of 2 6/5. What needs to happen is the state should get involved and have them step up their game . I spoke with the supervisor of the nurse's who is named above. Two days after she got there, someone changed her prescription medication for IBS from 3 times a day to as needed and gave her stool softener.
This caused her daily digestive distress.
We only learned of this the day before she was discharged.
There were 3 large cockroaches in the bathroom at separate times.
The nurses were very good to excellent.
She asked for a shower numerous times but only got one. A roommate got 2 or more every week.
The food was not up to par.
We were disappointed by the total package.

The nursing staff is uncaring except for one or two. The staff is overworked or at least they act that way. There is a lot of loud voices in the halls at night laughing and acting unprofessional. The nursing staff goes into patients rooms at night to have parties. When you need a member of the nursing staff you wait for hours upon hours to get assistance then you are treated like crap because of interfering with what ever they were doing. 
The food is fairly normal considering the type of facility. 

STAY AWAY from Port Orange Nursing & Rehab it is TOXIC.

I wanted to bring her home after a week and she was in no way ready…
After 2 weeks my mom could walk a whole lot better.
If I ever need another rehab I will definitely take her back to Port Orange Rehab.

I love the activities they have here. They keep me busy in an entertaining way, especially the live music. The nursing staff is good to excellent and the CNAs work hard to keep us comfortable. There is a beautiful tropical garden with a water fountain that I spend time in nearly every day and is quite relaxing. The food they serve to us is OK and it is sure nice to have it brought to us and picked up each day. The administrators are quite good at what they do. The Physical and Occupational Therapists are excellent and although it is hard work they make it fun for us. If I need help in the future I will be returning to Port Orange Nursing and Rehab Center. However he tested positive for Covid and was sent to another facility which is another story. This facility is clean and looks homey. The staff is friendly although on weekends staff is limited. He said food was better than other place so the weight he lost in the hospital he put back on here plus he could pick his menu which was a help in getting him to eat. When the patient gets to this facility staff meet with the family and patient to get to know each patient and their needs. Of course I had a couple of things I asked about during his stay but overall my Dad had a good stay and is home continuing to recover!!

                            [review] => My mother was admitted here status post a fall at home where she had broken her hip and pelvis. The card she received was minimal and mostly consists of the same complaints repeated in other reviews. The icing on the cake was when her condition deteriorated enough to be admitted to hospice.  She was visually seen by the hospice person and was accepted on Monday when I signed the papers.  She was to be formally evaluated the next day by the hospice nurse. I received a call from the hospice nurse at 9:30 Tuesday morning asking if the nursing home had called me. They had not.   My mother had taken a turn for the worse during the night and died around 9 am that morning. Not one person from the nursing home called us so my father did not get to say good bye. When I spoke to the DON I jjust got excuses instead of I’m sorry this happened.  The woman. In the office sat there eating Wendy’s chili as we spoke to her to close my mothers account. The lack of empathy and caring here is amazing  I would not even recommend this place to my worse enemy.

Added 1/24: Actually I did call to speak to someone and I believe it was the DON. Stacey. I spoke to the admin as well about a week later.  There was only excuses about why it wasn’t your policy to call a patient’s family once they admitted to hospice. My mom has not even been fully evaluated to be admitted yet, although yes, I had signed the papers the previous day. Someone could have called myself or my father that night when she got worse. Somebody from your facility could have called me when she died, but you didn’t. I found out a half hour after from the hospice nurse who had never even seen my mom.

                            [review] => As a health care professional and having a loved one here for the last week these are my observations. The rehab center staffs their facility at the minimum. There are not enough CNAs to provide care to patients who are unable to use the restroom. It takes 30-45 minutes for a call bell to be answered and if you are on the bed pan it takes another 30-45 minutes for a CNA to respond. If you need to go to the bathroom when the food trays are being handed out, your loved one is told we can't do it now you will have to wait until we are done in an hour. The RN/LPN is like a robot and only hands out medication, there is no autonomy or assessment. If a patient needs a medication in between the allotted medication hand out times, it can take 2 hours to get medication and that would consist of pain medication. If it's not scheduled it takes a long time to receive it. Being in the health care profession if I was not here, my loved one would end up urinating herself and not being changed. Some of the staff is very helpful and caring. Others act as if they are annoyed that the call  bell was activated. This place needs to be visited by the state.

One time she sat in her own poop for over 2 hours before someone came to help her. The lead physical therapist would even threaten to withhold therapy from her if she wasn't cleaned properly which is in no way her own fault. Even after my wife brought her concerns to the administrator, the neglect continued. Exceptionally long times for the call bell to be answered, horrible food, and horrible attitudes from the CNA staff. I wouldn't let my dog go here!

My 95 year old mother has been a long term resident at this facility for a year now.  We are grateful for the care she's been given, since being a long term resident on the East Wing.  Her Nurses and CNAs always treat her with kindness, care, patience, and top notch skill.  They notify me promptly when Mom has a fall, skin tear, or other problem, no matter how small.  The Activity Department, especially Leslie, is always attentive to Mom and has even sent me photos of Mom if we're out of town.  We visit Mom 3 days a week and the facility is always clean and pleasant to visit.  Other residents are friendly and appear to be as content as Mom.  Mom's clothes are laundered promptly, and she is always clean and well dressed.  Colleen, in the Salon, has done a fabulous job with Mom's hair each week.  Chantel has been a wonderful asset to us, right from the start.  Stacie and the entire PT,OT, and Speech Dept. has helped Mom retain as much of her physical abilities as possible and continue to monitor her.  We are grateful to have Mom is this facility and know that we can go about our own daily needs, knowing that Mom is well taken care of.

My mother-in-law  was given poor care during her stay for rehab. , following a hip repair.  After being there for 4 weeks she could barely stand, and pivot.  She developed a pressure sore on her foot on the operated leg, limiting her rehab.  It took a week for that to be addressed.  She was left in the wheel chair all day causing her leg to swell.  As her condition deteriorated  she was not assisted to eat.  I can not recommend this facility for elderly care.  It may be fine for someone who is alert and capable.

I have been doing physical therapy daily. The nursing staff dispenses in my medication in a timely manner. The food is good and you can order a substitute meal if needed.
It's the holiday season and the staff is cheerful but not overbearing. I would encourage anyone needing medical attention to contact the facility.

He was there for two weeks and they never helped him bathe.  All they would do is give him a damp cloth to wipe himself down.  Rarely changed his clothes, food of some sort stayed on the bathroom floor for a week and one of the attendants was rude and mean to him and would walk right by him to his roommate and never ask him if he needed anything. Food was not appetizing or appealing.  Rehab consisted of one 30 minute session a day.  He was laying in bed the rest of the time for the most part.  How do you learn to walk again like that.   We pulled him out and moved him to our hometown 12 hours away it was that bad.  He went from hell to heaven in the rehab center we got him here.  There has got to be a better place in the Daytona area than this place.  With all that said there were a couple of very kind nurses.  I think they were mentioned in another post.

Also, lots of very grumpy ppl here who seem to hate their jobs.  Beware they may not have time to return your call(s).

The best thing that happened at this place is we learned to avoid it.  A few people are excellent here including Chantel, Michelle, Karen.  It’s too bad one of them doesn’t run the place.

Lastly, when you see positive reviews here – notice that they are often from people connected to this place by employment and not written by patients or patients’ family members.   These people are not “customers”  so why are they leaving reviews unless they were asked to? Smh.

My Grandmother was placed  at this facility for approximately one month and it was nothing but a nightmare from day one. We had no communication from the staff about her care and overall health, which is pretty essential when you cannot visit the facility due to COVID lockdowns. We trusted this facility and staff after careful consideration and much research on nursing homes in the area, only to majorly regret this decision four weeks later when she passed away. The nursing staff was rude, paid little to no attention to the residents, and even left my grandmother in a wheelchair for four hours and unable to get back to her bed after "forgetting" to take her for a walk after getting her up. They then tried to deny my family to be at her bedside in her final hours due to COVID restrictions, even though we had already been there the previous 24 hours. There was no consistency in guidelines from day to day and the care she received was subpar at best. She was very unhappy and upset everyday when we spoke to her and we could never get anyone from the facility on the phone to talk about what was going on. She would tell us she hits the call button in her bed when she needed help and nobody would ever come to assist. She was bed ridden at this point and heavily relied on the staff for assistance. While on the phone with her one evening, I even witnessed an aide scold her for calling for help and bothering them, which is what they are getting paid to do and the whole reason she was there!! I would not trust my dog to the care of this facility, let alone another family member.

  I overheard a couple of Family members of a different resident complaining about some petty issues. I put some thought into what they were saying and questioned the validity. It occurred to me that there was definitely a difference in the care that my dad was receiving in the ICU wing of the local hospital versus the care at this rehab facility. I recognize the importance of understanding that there is a huge difference in the mission of each facility. For what Port Orange is here for... They are knocking it out of the park.                             [review] => My mother in law (MIL) was at this facility. I was her Heathcare Surrogate. The weekend supervisor, Dorothy, mocked my mother in law when , she told her she was short of breath. She stood at the foot of her bed in the rehab section, she started breathing in and out as if she were gasping for air telling my mother in law, that if she were short of breath shed be gasping for air as she had demonstrated. My mother in law had congestive heart failure and when the fluid would build up around her heart she became very short of breath. Dorothy behaved like that in the presence of an independent caregiver there to help my MIL. I was called and I spoke to Dorothy as she was refusing to send my MIL out to the hospital. I insisted that she be taken to the ER as it was a weekend and her General Practitioner's lab where she could be treated  My MIL could have easily drown in her own fluid due Dorothy's negligence. in the ER they administered a diuretic immediately. 

A few months later I was no longer assisting my MIL and she had to go to the hospital and then rehab. My MIL liked Port Orange Rehab's music and she didn't remember her previous bad experience. 

I then began helping my MIL in the last 5 months of her life. Again she went into the ER then they admitted her and she again returned to Port Orange Rehab against my advise. While there she was having fainting episodes and the PA took her vitals and said my MIL didn't need to go to the ER. Someone on staff overrode the PA and they sent my MIL to the ER. My MIL proceeded to have 2 heart attacks that night in the ER  from which she suffered brain damage and she then had to  go on a ventilator. My MIL died 2 days later. 

I don't blame Port Orange Rehab for the heart attacks or subsequent death, however I do know that the first time when  Dorothy mocked her and wouldn't send her to the ER she could have died by drowning in her own fluids, because Dorothy said she could wait until Tuesday to see the PA. I had to insist that my MIL be taken to the ER. 

The second time the PA, an Asian woman, did not send my MIL to the ER. Based on the syncopal episodes she was experiencing, she should have been taken immediately to the ER. 

This facility is terrible in identifying critical health issues that cannot wait days for to see the in house PA.

Don't take your loved ones to this rehab facility if you want them to come out alive.. 

Therefore, I do not think that Port Orange Rehab is a safe and comfortable environment for anyone trying to rehab.

                            [review] => My 80 year old mother spent 2 months in the Port Orange Rehab Center recovering from complete shoulder replacement after falling in her home. Overall, I am pleased with the care she was given during her stay. She road out the Irma Hurricane at the PORC. The power went off, but the generators were up and running very quickly. The staff was usually responsive to her needs. The food was very institutional but adequate. Mom's biggest complaint was there were too many potatoes at every meal. Rehab is an unpleasant experience but my mother received adequate care and is now back home. No potatoes to be served for a while.

                            [review] => First off, I have never felt compelled to write a negative review about any establishment, but this hell hole takes the cake. Where to even began... the nursing staff is highly unprofessional, and more interested in their phones than the care and well being of their patients. Physical therapy placed my grandmother in a chair as part of her treatment after she sustained a pelvic and sacral fracture. After informing staff that the pain of sitting was unbearable and she was about to pass out, she was told to sit there quietly. My grandmother may be in her 80s, but she is very "with it" and in no way unable to comprehend or make decisions for herself. I was on the phone with my grandmother who set the phone down unbeknownst to her nurse. I listened to the nurse mockingly correct my grandmother after my grandmother informed the nurse she has had a temperature over the last two days. The nurse went on to tell my grandmother that she has no documentation of temperatures and that she just informed her doctor on the phone that her vitals are perfectly normal. While still listening on the phone, another nurse comes in and takes her temp which he says is 101.2. The nurse then proceeds to try to convince my grandmother all the recorded "normal temperatures" where taken from under the arm. My grandmother swears no one has ever put a thermometer under her arm. That night after reporting what had happened to my family, she was diagnosed with pneumonia in the ER. If you want your loved ones to be completely disregarded and discredited because they are "old" then Port Orange Nursing and Rehab is the place to send them, but just know they'll probably get pneumonia and die after a few weeks because the nursing staff can't be bothered to actually record their vitals.

I will not recommend this place for anyone that have a family in need of rehab services, please due your research before admittting your love one to this place.
The staff gets upset for what they are getting paid to do as a career they chose. They do not have the heart or passion to do this job!!! I forgot to mentioned the therapist Irma was cruel pushing my mother to do more than what she can handle. Also the staff are negligent about bathroom needs.My love one has suffered to the point that does not want to ask for help, due to their rudeness and lack of professionalism.
Please DO NOT even consider or give this place a chance.

                            [review] => Experience was not a pleasant one.  My mother was there for rehab due to a blood infection and a wound on her foot.  She also had memory issues, and I would call and ask to speak to someone and anytime they didn't know the answer to my question, they said they would get back to me and never did.  One day my mother called me crying because she was in so much pain and they were not doing enough for her.  I asked several times to find out who her wound care doc was, and never received an answer.  She was left one night on the side of her bed for a few hours.  When someone walked by she told them that she was cold and they asked her what she wanted them to do about it and she said a blanket would be nice, and they picked one up off the chair and threw it at her and walked out.  She was transferred to hospital a couple of times, with the last one going from hospital to hospice and passed away a week later.  When we came in to get her belongings out of the room, only one person came in to see what we were doing and no one said anything to us.

I left my number and information several times for a nurse and/or doctor to call me back (I am not in the area) regarding my mother's care.  I never received one phone call back.  One day my mother called me crying that she was in so much pain, again tried to speak to someone and only did because they came into room and mom handed phone to them.  They also didn't have her meds correct, never knew who her wound care doc was and have a picture of what her foot (with wound) looked like - it was 1/2 covered and 1/2 exposed (and she was just getting over a blood infection).  She also had an issue one night and was left sitting on the side of the bed for several hours.  Someone finally walked by and she asked for a blanket and they threw it at her and said "there" and walked out of room.  I complained about this and said I wanted an investigation into it and again, never received a phone call.

                            [review] => This place is very impressive to family and visitors, because it's daytime. I was in Rehab in November 2014. Therapy was great, though way too overcrowded.  Meals were ok. Once the doors locked at night, the place turned into a nightmare. I had a LPN threaten me at 2am, while I had no roommate to witness. I was told that I might not want to rock the boat while was here. Then refused to let me reply, kept telling me she wasn't finished. Spoke with Administrator, who did nothing. Went to another facility in Daytona, SO much better.

Transferred her to another facility and had her back talking and feeding herself in 24 hours. Almost lost the Medicare days for rehab because of this place. Most CNAs were nice but overworked not enough of them for the patients. Dirty laundry left in the room and gloves on the floor. I would give 0 stars to this place! The admissions lady could have cared less about anything we talked to her about. My advise would be to do more research on a place before you place a loved one in any care center.

                            [review] => Although there are exceptions to this, my father's experience with the nursing staff is not a pleasant one.  He is there for rehab and is in his 60's so not elderly, no loss of memory or thinking skills whatsoever.  He is an engineer so is very intellegent.  They not only offered him but argued with him to take medication that was not his.  He knew what meds he was to take and they brought an additional pill that he should not be taking nor was he prescribed.  He was told that it was in the computer and they then reported that he would not take his meds.  When confronted he re-stated that the pill he refused to take was given to him by mistake and was not his.  He continually waits 45 minutes or more for help when he pushes the button for assistance.  Keep in mind when we visit him, the nurses had plenty of time to chit chat while you could hear buzzers going off all over the place for extended periods of time of people that needed help.  When confronted, the nurses seemed annoyed that they were bothered.  The nursing staff also refused to give him his discharge papers from the hospital.  I believe this is illegal.  He filed a formal complaint and was then told by his nurse that they were going to discharge him.  Something is not right with this place.  Do not send your loved one there.
