Our family was very pleased with the care our dad received at this facility. The facility was clean, therapy department was wonderful and so were all the staff. He is doing great as his assisted living facility. Thank you.

I was discharged sept 3rd after 3 weeks of great.physical therapy. Clean and bright. Most of the nurses and CNAs were excellent and treated the patients well. The discharge planning was handled very poorly by social services. I still...after 4 weeks...have not received any follow up inhouse physical therapy sessions. And the Dr who saw me at the facility has not called or contacted me.

Words cannot fully express the gratitude I feel for the exceptional care my mother received at Port Orange Rehab. The staff were not only attentive to her needs, but also compassionate, patient, and always available to answer my questions. The therapy department was outstanding, and while I wish I could remember each of their names, please know their efforts left a lasting impact. The front office staff consistently welcomed me with warmth, making every visit feel comfortable and reassuring. From the nurses to the aides, every team member took the time to ensure my mother’s comfort, which I believe played a tremendous role in her recovery. Thanks to their dedication and kindness, my mother is now home and doing wonderfully. I am truly thankful to Port Orange Rehab and every member of the team who made this journey a positive one for both my mother and our family.

I recently spent 3 weeks at the Port Orange Rehabilitation & Nursing Center. My stay there was as pleasant as could be considering I was in great need of rehabilitation. The nurses and CNA’s were always so pleasant as they were taking care of my needs. I desperately needed physical and occupational therapy and I found the staff in rehab very knowledgeable and they helped me progress as quickly as my physical shape allowed. In addition they were always cheerful and rehab was a positive place to be even though there was “pain” in the process. I must add that the food served was great. The main chef is a great cook and served a lot of “comfort” meals. He also aimed to please and one could order substitutes if the printed menu items were not favorites. My only advice would be to do all you can to make sure your follow up care is instituted in a timely manner. I was in this center for several weeks and returned home feeling very thankful for the care I received at Port Orange Rehabilitation & Nursing Center.

My dad was at this facility for 3 weeks following hospitalization for a fall. With few exceptions everyone I had contact with, from the receptionists, to the maintenance workers, to the nurse aides were very pleasant and kind to the residents. When i had concerns about my father's condition the medical staff were very responsive. My dad also really liked the therapists. The administrative staff were really helpful in guiding me through the discharge process so he could transfer into assisted living. The facility was very clean. I felt my dad was very well taken care of.

I actually took my husband out of one facility, brought him here, and I couldn't be more pleased. The staff is caring, pleasant, patient, and very professional. The facility is very clean, the food is good, and everyone is very helpful. I feel confident he's in good hands.

Being in a nursing home is an uncharted experience for most families, but at Port Orange it has been a great transition for my loved one and our family. Robert in Social Services and Caitlyn in activities has been a big part of making this easier. The administrator Ashley is very responsive and involved in all aspects of care.

This center really made me feel comfortable leaving my family member in. The nurses are caring, compassionate, and most importantly, competent. Staff looks happy which makes me feel as if management treats then appropriately. Of course no one ever wants to have to use a nursing home for their family but I'm so glad I was able to find one that I feel is truly good. Definitely recommend!

I can honestly say this facility is one of the best facilities in the area. They have a proactive approach to your loved one's care. Upon entering the building, you will be greeted by Christina, the receptionist where she will give each visitor in your party a badge. I know sometimes there may be a line and you might have to wait, but that is only to protect you and your love one, in the event of an emergency, the facility has an accurate count of staff, visitors, and residents. The doors stay locked. You can press a buzzer to enter, collect a badge, and visit your loved one. Keep that badge because you will need it to get out. Visiting after hours, no problem. They have an additional buzzer, that goes directly to the floor. You might have to wait a minute because the staff member has to walk down to the front to let you in. When your loved one arrives for admission, they will be greeted by a nurse and a CNA. Be patient. It may take some time. This facility is busy. On average they are getting two to three admissions a day. A nurse can have anywhere from 1 to 40 patients while a CNA can have anywhere from 1 to 20. I can assure you this facility staff's well above the STATE guidelines. The CNA may have anywhere from 1 to 15 on a bad day and the nurse may have anywhere from 1 to 30. You may hear a staff member from time to time. Say we are working short today, That's not true. They are just spoiled because usually they have anywhere from 1 to 10 patients. Your loved one will get 3+ meals a day. There is a selective menu that they can fill out a week in advance. If their meal tray arrives in the room and they don't like it, they can always ask for an alternative which usually includes grilled cheese, chicken tenders, a hamburger, or many other options. Tony, the dietary manager will come around and get your loved ones likes and dislikes. And while we still have to follow your loved one's diet, Tony will certainly try to include their likes. I've seen Tony go as far as making special meals for patients who are refusing to eat or losing weight. Your loved one does not have to just lay there in the bed like a hospital. They do not have to just stay in their room. There is a wonderful activities department that is ran by Caitlin , Shannon, and Shirley. Everyday these ladies have some type of fun entertainment like live music, bingo, popcorn in a movie, coloring, arts and crafts and much more. This facility has one of the best therapy departments I've seen. They have physical therapy, occupational therapy and speech therapy. All three departments. Have a wonderful therapy manager named Stacey. Stacy has been with the company for over 20 years. If you like the therapy, your loved one is receiving in the facility, once they are discharged you can bring them back for outpatient therapy. Looking to arrange a discharge? You can talk with Bob or Candice from social services. They usually set everything up for you. Whatever equipment you need upon discharge, to your next doctor's appointment. Want your loved one to stay long-term care or transfer to another facility. They can help with that too. Missing an item or have a complaint. Have a staff member help you fill out a grievance. They usually find and return the item quickly. Or as long as it's on your loved one's inventory sheet, the facility will cover it. Trayshawn in the business office is good at notifying you of your loved ones co-pays or insurance updates. She can also help with the Medicaid application process. The management team here do a phenomenal job. They ensure that your loved one has the best care possible and that all policy and procedures are being followed. However, they are not mind readers. They do not know something is going on unless you speak up and tell them. I can assure you they are quick to resolve whatever issues you or your loved one is having. My time has come to an end at this facility, by my own choice, but I can assure you, your loved one is in good hands!

I work closely with Candace a case manager to help patients with durable medical equipment and she is always a pleasure to work with. She works very hard to obtain the documentation needed in order to bill the patient's insurance so out of pocket cost is low to none.

The nurses and staff are attentive and treat loved ones like family. Floretta, and Maureen, always smile and are as friendly as it gets, When you walk in there is always a warm smile from Nicole.and usually Maureen is there with an enthusiastic hello. Nurses Beth, Cheryl, Nyree, Rodney, Laurie and other are all great and the therapy staff is great. If we ever had an issue Justin and his team were quick to listen and implement change.

During the time my wife was a patient there for 180 days overall the care was very good to excellent depending on the treatment provider. Considering the length of stay there will be both good and not so good experiences but neither should solely determine the final result of returning home and feeling better. It seemed that the CNA’s who carried a lot of the workload had to many patients and not a lot of time to check on them but once or twice during a shift. However, they did fine. Tho PT/OT staff was outstanding and deserve much credit for their commitment and professionalism in restoring her functionality. Over this lengthy stay we met and interacted with so many wonderful and caring staff that to try and name all and miss one would be a disservice to the omitted and an embarrassment for us. My wife became very ill and was placed into the Port Orange Nursing and Rehab facility where she remained for over a year before passing. I was satisfied over all with her care and am great full for the care she received from Nurse Jennette and Hospice Nurse Tim. Most of the staff were courteous and caring and I thank those who attended her. Overall, the facility is clean, the food was adequate and would recommend for those in need to consider the facility.

My dad spent 20 days at this facility the staff were all very attentive listened to what my dads needs were & followed through the entire stay. Food was good not what you’d expect from a rehab facility. This place is the absolute finest of the finest! Donna V. and her team there are so wonderful. We had to place my mother in a long term facility and after visiting many different bad facilities of no help, smelly halls, mean staff, and unhappy residents, we came to visit Port Orange Nursing and wow! It was so overwhelming with lots of love not only from the staff but the residents there are happy too! When you walk in, you can feel the a/c, theres no smell of urine, the staff is smiling, and….. the residents are extremely happy. What a slice of heaven! I would highly recommend this facility to anyone considering or needing to place their loved one in special care. Thank you Donna for such a great experience!!!

I have updated my review simply for the staff. Everyone they have on staff is very good. Rehab staff is excellent. There should be a curfew for TV’s. Some patients are hard of hearing and have their tv’s up loud. Even with closing my door it was still loud. Some of the patients are asleep and tv still going. Different channels on loud. Very hard to sleep. Last night there hubby brought me ear plugs. Thank you to all the CNA’s.

My mom was recently in Port Orange Nursing and Rehab. The staff was fabulous. They took excellent care of my mother. I did find out the staff knew more than I did about her care. I wanted to bring her home after a week and she was in no way ready… After 2 weeks my mom could walk a whole lot better. If I ever need another rehab I will definitely take her back to Port Orange Rehab. I will also recommend them to my friends…

My experience at the rehab center was absolutely fantastic the nursing staff has been wonderful I've been here for two and a half months and they've treated me with nothing but respect places clean and well efficient I recommend it to anyone who needs help with rehab and or nursing care. I was referred to Port Orange Nursing and rehab center by the hospital. I'm fortunate enough to have used their facility twice and I'm a better and ;healther man because of their help and expertise. The entire staff does their best to make you feel .comfortable and well taken care of. The rehab program is excellent and I spent a lot of time. there The facility is clean and the rooms are nice and the food is good. The chef sends a weekly sheet for you to alter your menu for your own preference. A very easy place to refer to any friends or relatives that need help after they're out of the hospital. Alex

I recently had the pleasure of assisting the activities department during their outdoor workouts in the garden. I loved being a part of this event and thoroughly enjoyed getting to know some of the amazing residents. It was refreshing to see how much the staff really cares for all of them. It was an honor and I can't wait to do it again. Seeing the smiles on their faces reminded me of my own grandparents.Thank you so much for including me.

I had the honor of tuning their piano, as well as, playing piano for one of their special events. The residents enjoyed a lovely “50’s” themed environment while eating dinner. I have been a resident at Port Orange Nursing and Rehab for one year. For the most part it has been a very positive experience. I love the activities they have here. They keep me busy in an entertaining way, especially the live music. The nursing staff is good to excellent and the CNAs work hard to keep us comfortable. There is a beautiful tropical garden with a water fountain that I spend time in nearly every day and is quite relaxing. The food they serve to us is OK and it is sure nice to have it brought to us and picked up each day. The administrators are quite good at what they do. The Physical and Occupational Therapists are excellent and although it is hard work they make it fun for us. If I need help in the future I will be returning to Port Orange Nursing and Rehab Center. The nurses and CNAs were really good. The Therapists for Physical and Occupation as well as the Speech Therapist were fantastic giving you the attention and care you needed to be able to get back to your everyday normal life. My only complaint is that the food seems to be sub par on quality and quantity. They seem to run out of some items.

My Dad fell and after being in the hospital for 8 days needed a rehab facility I chose PO rehab since he had been there before and it was close to home. However he tested positive for Covid and was sent to another facility which is another story. This facility is clean and looks homey. The staff is friendly although on weekends staff is limited. He said food was better than other place so the weight he lost in the hospital he put back on here plus he could pick his menu which was a help in getting him to eat. When the patient gets to this facility staff meet with the family and patient to get to know each patient and their needs. Of course I had a couple of things I asked about during his stay but overall my Dad had a good stay and is home continuing to recover!!

My husband was discharged from hospital so soon I did not have enough time to arrange much. I chose PONursing on recommendations. We were not happy with the only room available but THE CARE was more than expected. Almost Everyone was kind and helpful to explain and answer our questions. This made such a difference in our experience. We arrived on a Holiday weekend but still found the care needed. He was moved to a better room and we are well pleased with his care. Update, we made our own Urologist appointment and had help putting him in our car. I couldn't manage getting him into Dr Morrow so his PA kindly assisted me to get Jerry into the Doctor and back in the car after he was seen. We decided it was best after 12 days in rehab to go home and have Home nurse and Therapy.

I am happy to give Port Orange Nursing and Rehab Center 4 stars. After a long, drawn out bookkeeping issue, I did not wish to give them 5 stars. My 95 year old mother has been a long term resident at this facility for a year now. We are grateful for the care she's been given, since being a long term resident on the East Wing. Her Nurses and CNAs always treat her with kindness, care, patience, and top notch skill. They notify me promptly when Mom has a fall, skin tear, or other problem, no matter how small. The Activity Department, especially Leslie, is always attentive to Mom and has even sent me photos of Mom if we're out of town. We visit Mom 3 days a week and the facility is always clean and pleasant to visit. Other residents are friendly and appear to be as content as Mom. Mom's clothes are laundered promptly, and she is always clean and well dressed. Colleen, in the Salon, has done a fabulous job with Mom's hair each week. Chantel has been a wonderful asset to us, right from the start. Stacie and the entire PT,OT, and Speech Dept. has helped Mom retain as much of her physical abilities as possible and continue to monitor her. We are grateful to have Mom is this facility and know that we can go about our own daily needs, knowing that Mom is well taken care of.

Both of my elderly parents were residents for a collective 15 years. Throughout that time they received thorough and loving care from the staff. The activities Director is an example of going beyond the call! The facility, inside and out, was always meticulously clean. I was always immediately contacted by a nurse when their was an issue with my parents health. I highly recommend the facility to entrust your family member's health care needs.

I have been a patient here for a week now, with severe neuropathy in my legs. Since my admission the entire staff treated me like I was part of the family. I have been doing physical therapy daily. The nursing staff dispenses in my medication in a timely manner. The food is good and you can order a substitute meal if needed. It's the holiday season and the staff is cheerful but not overbearing. I would encourage anyone needing medical attention to contact the facility. Lastly, supervisor Wendy helped me with a special request. It's a great place to get it together and regroup

My father was a guest at Port Orange Nursing and Therapy for approximately two weeks. Overall we were very happy. Very clean facility with continuous cleaning occurring. Visiting hours set with Covid in mind. Very friendly and helpful staff. Professional yet friendly.... Always had time for a quick chat with the people staying there. Knowing a lot of elderly people need that little extra chit chat or reassurance... they took the time instead of just brushing them off. That meant a lot to me ! If my dad had to be somewhere for pt/ot ... I was glad he was there.

The room was sufficient. The staff was very accommodating and helpful. I was in for only 4-5 days, but had a pleasant enough experience, given the fact that I had broken my leg. Covid made some tasks more difficult for the staff. There were a few delays in medication timing...but in general, my feeling was that the staff was doing their very best, conscientious of their duties, and asked frequently if there was anything they could do for me. I would go back again, if rehab was needed. My nurses and support staff were kind and caring.

Great, friendly staff. I was very comfortable having these folks charged with caring for Papa B. The physical\occupational therapy is top notch. They did not let my dear old dad get away with squat! They push him as far as he could go with a lot of love and support. I overheard a couple of Family members of a different resident complaining about some petty issues. I put some thought into what they were saying and questioned the validity. It occurred to me that there was definitely a difference in the care that my dad was receiving in the ICU wing of the local hospital versus the care at this rehab facility. I recognize the importance of understanding that there is a huge difference in the mission of each facility. For what Port Orange is here for... They are knocking it out of the park. I do not have any suggestions on how to make the place better. Thank you for focusing on your mission.

This was only a temporary situation for my mother. It seems to be like a few other nursing homes in the area. Communication is not great, but better than at the last rehab center my mother visited. This place is leaps and bounds above that place (which I won't mention by name at my mother's doctor's request). I like the outdoor fountain and gardens which is a nice area to take loved ones during visits.

This is one of the nicest skilled nursing and rehab facilities I have ever seen. The staff work very hard and care about the patients. It is always clean and has a pleasant atmosphere, a beautiful tropical garden area, and lots of live music for entertainment.

Port Orange Nursing and Rehabilitation was wonderful. The rooms were very nice. My mother-in-law was in a shared room. She did not like the food that much. They had ice cream parties, and there were people coming in to sing. They had arts and crafts. It was clean. There was no smell. The people working there were very nice.
